CCB Credentials Form
The CCB Bylaws require that chapters and Statewide Affiliates submit their credentials and rosters to the CCB office.
Members listed on rosters with less than complete roster information, will not be counted. Please send rosters to CCOTB@ccbnet.org.
* Please Note: This reporting requirement is to be met by all chapters and statewide affiliates whether or not you are going to send anyone to the convention. It is a component of keeping your chapter or statewide affiliate in good standing as an affiliated organization with the California Council of the Blind and the American Council of the Blind.
