2025 Chapter and Affiliate Officers Information Form

It is time to submit the officer information sheets for your chapter or statewide special interest affiliate. We need this information as soon as possible in order to properly update the CCB web site and presidents email list. The deadline for submission of this information is noon on February 18th.

Please update this form after each election on changes in officers and/ or Presidents' Listserv designees, Membership contact and chapter meeting location information for the CCB webpage. This form must include each officers' name, phone number, mailing address, and email address if any. Please note that the President, Vice President and Membership contact name, email and phone number will be published on the website.

Note: We have added a new section to this form for designating a CCB listserv liaison for your chapter or affiliate. The listserv liaison will be a contact person that will notify the Technical Operations Group (TOG) when listserv maintenance is needed. I.e. adding/removing people from your email list.
